OK, so it may be a little outdated as this law was bought in back in 1992 - way before the internet and easily accessible GPS, but it's still pretty relevant.
The Control of Dogs Order 1992 states that 'any dog in a public place should wear the name and address of the owner either inscribed on the collar or a name plate or disc attached to it’.
Here’s a suggested example of what is required on an ID dog tag:

OR

In both examples, your telephone number is optional, but given most of us own a smartphone we would highly recommended it.
